ABSTRACT

Objective@#To analyze the epidemiological characteristics of infectious diseases at schools in Beijing from 2010 to 2020, providing evidence for the prevention and control strategies in school infectious diseases.@*Methods@#Information on public health emergencies was collected from Public Health Emergency Reporting System (the subsystem of Chinese Disease Prevention and Control Information System) reported from 2010 to 2020.@*Results@#A total of 146 public health emergencies and 138 public health emergencies of infectious diseases in schools were reported in Beijing from 2010 to 2020, including 4 291 cases with the rate of 2.32% and affected or exposed 185 179 cases. There were significant difference in mean annual incidence rates( χ 2=782.46, P <0.01). There were 71 events of respiratory infectious diseases and 66 events of intestinal infectious diseases, accounting for 51.45% and 47.83%, respectively. The annual incidence peaks of public health emergencies of infectious diseases were during March-June and October-December. The events mainly occurred in kindergartens and primary schools among each stage of school periods with 51 and 46 incidences respectively, which accounted for 70.29% of the total number of public health emergencies in schools. The leading infectious diseases among all the reported events in kindergartens and primary schools were hand foot mouth disease and varicella. Varicella and other infectious diarrhoeal diseases were at the top lists of infectious disease outbreaks at the secondary and university stages.@*Conclusion@#Infectious diseases events were the major type of public health emergencies at schools in Beijing from 2010 to 2020. Respiratory infectious diseases and intestinal infectious diseases were the keys to the prevention and control of public health emergencies related to school. It is necessary to strengthen the surveillance for public health emergencies especially for symptom surveillance. The prevention and control measures should be taken according to the characteristics of different age groups. At the same time, the prevention and control of school infectious diseases and the drill of the plan during peak periods need to be particularly strengthened.

ABSTRACT

Objective:To summarize the nursing experience of a patient with refractory hypoxia, septic shock and delirium after pulmonary endarterectomy combined with coronary artery bypass grafting for severe chronic thromboembolic pulmonary hypertension.Methods:An elderly patient with severe chronic thromboembolic pulmonary hypertension admitted to China-Japan Friendship Hospital in December 2020 was given nursing measures in terms of hemodynamic management, anticoagulant management, nursing of intractable hypoxia and balloon pulmonary arterioplasty, infection prevention and control, lung rehabilitation, early functional exercise and transitional nursing etc.Results:After multiple consultations with multidisciplinary experts inside and outside the hospital, the patient′s diagnosis and treatment plan was refined. After individualized and comprehensive nursing, the patient recovered well after surgery and was transferred back to the common ward 57 days after surgery, and discharged successfully 73 days after surgery.Conclusions:The individual and comprehensive nursing measures for this patient have effectively improved the prognosis of the patient and improved the quality of life of the patient.

ABSTRACT

Recent studies have shown that tumor microenvironment (TME) is significantly different from normal tissues, such as the change of abnormal enzyme expression, hypoxia, pH and so on. TME is closely related to the induced proliferation of tumor cells, angiogenesis, immune escape and the production of multidrug resistance. Cancer associated fibroblasts (CAFs) are the main stromal cell types in the TME. A variety of cytokines secreted by CAFs play an important role in tumor growth, invasion and metastasis. Understanding the role and mechanism of CAFs in the development of breast cancer is expected to provide a new target for improving the prognosis of breast cancer patients.

ABSTRACT

Objective:To evaluate the feasibility of esophageal pressure (Pes) calibration by the esophageal balloon pressure-volume (P-V) curve during assisted mechanical ventilation.Methods:A prospective study was conducted. The postoperative patients admitted to intensive care unit of Beijing Tiantan Hospital Affiliated to Capital Medical University from June 2017 to January 2019 who needed pressure support ventilation by tracheal intubation and Pes monitoring with stable breath were enrolled. The Pes monitoring was performed by the esophageal balloon with a small geometric volume (2.8 mL). ① Balloon volume tests of esophageal balloon were performed by inflating intermittently 0.5 mL increments up to 2.5 mL, the end-expiratory and end-inspiratory Pes were recorded to obtain end-expiratory and end-inspiratory P-V curves. The intermediate section in end-expiatory P-V curve that showed linear correlation was identified (as intermediate linear section), whose volume range was balloon working volume ( Vwork) and slope was esophageal wall elastance (Ees), the balloon volume with the largest difference between end-expiratory and end-inspiratory Pes was the best balloon volume ( Vbest), and the product of Ees and Vbest was esophageal wall recoil pressure reacting to balloon filling. To minimize the effect of esophageal wall on Pes, the calibrated Pes was the difference of Pes and esophageal wall recoil pressure. The consistency of calibrated Pes obtained by balloon volume at Vbset and other Vwork were analyzed. ② For the convenience of clinical application, a simplified method was introduced to calibrate Pes. Based on all Vwork of patients located in 0.5-1.5 mL, the difference of end-expiratory Pes between balloon volume at 0.5 mL and 1.5 mL divided by 1.0 mL was used to estimate Ees, and the Pes among 0.5-1.5 mL was calibrated by Ees obtained by the simple method. The consistency of calibrated Pes obtained by the simple method and standard method were observed. Results:Totally 30 patients were enrolled, all end-expiratory and end-inspiratory P-V curves existed the intermediate linear section, the calibrated Pes at Vwork did not increase with the balloon being inflated and had a good consistency with the calibrated Pes at Vbest, mean difference and 95% confidence interval (95% CI) was -0.02 (-1.50-1.50) cmH 2O (1 cmH 2O = 0.098 kPa). The Ees and calibrated Pes estimated by the simple method had a good agreement with the standard method, mean difference and 95% CI was -0.2 (-1.0-0.6) cmH 2O/mL and 0.2 (-1.1-1.4) cmH 2O, respectively. Conclusions:During assisted mechanical ventilation, the use of a small geometric volume esophageal balloon to monitor Pes and balloon P-V curve to calibrate Pes is feasible. The simple method can be used for simplifying clinical application, that's only by monitoring Pes at balloon volume at 0.5, 1.0 and 1.5 mL to evaluate the Ees and calibrate Pes.

ABSTRACT

Esophageal pressure monitoring provides a minimally invasive method to assess the pleural pressure, which can be used to differentiate the lung and chest wall mechanics. The information of transpulmonary pressure, work of breathing, intrinsic positive end-expiratory pressure and respiratory muscle performance can facilitate the proper setting of mechanical ventilation. Esophageal pressure monitoring is still not routinely used in the clinical setting because of difficulty in esophageal balloon catheter placement and data interpretation due to esophageal pressure monitoring has certain technical requirements, and the measurement results are influenced by many factors such as airbag volume, location, esophageal wall elasticity and mediastinal organ weight. In this review, we introduced technique for esophageal pressure measurement and calculation of transpulmonary pressure aiming to promote the clinical application of esophageal pressure monitoring.

ABSTRACT

Objective To investigate the effect of different composed combination of EGCG and cisplatin on human colon adenocarcinoma cell line SW620 and the related mechanisms. Methods SW620 cells at exponential phase were treated with cisplatin(5 mmol.L-1)and EGCG(0, 25, 50 and 100 mmol.L-1).Seventy hours after the cultivation, cell proliferation was detected with MTT;the percentage of apoptotic cells was determined by flow cytometry;expression levels of JAK2, p-JAK2, p-STAT3, STAT3, Bax, Bcl-2, cleavage-PARP, and cleavage-caspase3 were detected by Western blotting. Results The SW620 cell proliferation rate was(95±5)%,(67±5)%,(42±5)%(22±4)% and the apoptotic rat was 5%, 15%, 35%, 57%in the 0, 25,50, 100 mmol.L-1 EGCG groups, respectively.Moreover, EGCG up-regulated the expression of Bax, cleavage-PARP and cleavage-caspase3(P0.05). Conclusion EGCG can promote cisplatin-induced apoptosis and proliferation inhibition of human colon adenocarcinoma cell lines of SW620 by suppressing JAK2/STAT3 gignaling pathway.

ABSTRACT

Objective To investigate the regulation role of gastric cancer related miR-148a on gastrin receptor CCKBR expression, and find the correct binding sites of miR-148a in CCKBR 3’UTR.Methods The potential binding sites of miR-148a in the CCKBR 3’UTR were predicted with the bioinformatic tools;The miR-148a expressing plasmid was constructed by PCR, and miR-148a expression was verified by Northern Blot;The luciferase report plasmids containing the wild type and mutated binding sites of CCKBR 3’ UTR were constructed, and were used to study the regulation mechanism and identify the binding sites of miR-148a by luciferase activity analysis; The regulation effect of miR-148a on CCKBR protein expression was checked by Western Blot.Results Three potential binding sites of miR-148a in the CCKBR 3’ UTR were found; The miR-148a expressing plasmid was constructed successfully, and highly expressed miR-148a after transfected to gastric cancer cells;The inhibitory effect of miR-148a on CCKBR protein expression was checked by Western Blot.Over-expression of miR-148a inhibited CCKBR expression by directly binding to the binding site in CCKBR 3’UTR 423bp.Conclusion CCKBR is a target of miR-148a, and its expression is inhibited by the binding of miR-148a on its 3’ UTR, indicating that miR-148a may participates in the progression of gastric cancer by regulating CCKBR expression.

Objective To summarize the successful experience of radiodiagnosis specialist nursing model in our hospital.Methods Retrospectively analyze the course of establishment and development of radiodiagnosis specialist nursing model in our hospital.Many aspects including setting of the nursing unit,post,quota and salary,development of post responsibilities,systems,standards and procedures,establishment of quality control and performance evaluation system,standardization of the conventional specialist technology,study and application of the new specialist technology were expounded.Results Radiodiagnosis specialist nursing model had been established and improved after five years' exploration.Quality index of care,service and safety showed a steady upward trend.Specialist new technologies had prominent features.Teaching and research achieved initial success.Conclusions Establishment of radiodiagnosis specialist nursing model makes radiology nursing connotation more comprehensive.More importantly,it is of significance in guiding the development of radiodiagnosis specialist nursing.

ABSTRACT

<p><b>OBJECTIVE</b>To optimize the combination modes of medical materials in extract of Huanglian Jiedu decoction.</p><p><b>METHOD</b>Divided the medical materials of Huanglian Jiedu decotion into 15 groups and extracted by semi-bionic estraction (SBE), the contents of berberine, baicalin, geniposide, total alkaloid, total flavonoid, total integral calculus area of HPLC and extract of 1 000 were determined, standardized, weighted and evaluated, respectively.</p><p><b>RESULT</b>The maximum evaluated value Y for the extracting technique was AD + BC.</p><p><b>CONCLUSION</b>The optimal combination mode of the perscription is that Rhizoma Coptidis and Fructus Gardeniae, Radix Scutellariae and Cortex Phellodendri Amurensis are decocted together respectively then the two decoctions are merged.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effects of the solid-state fermentation technology in extracting the ingredients of traditional Chinese medicine (TCM).</p><p><b>METHOD</b>Using Danggui Kushen pills as an example, four index ingredients of ferulic acid, matrine, matrine alkaloids and dry extract were compared comprehensively, and the difference between the two water extractions of fermented pills and unfermented pills was evalued.</p><p><b>RESULT</b>The comprehensive evaluated result was that extraction of fermented pills was better than that unfermented.</p><p><b>CONCLUSION</b>The solid-state fermentation is conducive to the extraction of prescription of Danggui Kushen pills.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To optimize the extraction conditions for semi-bionic extraction of Guizhi Fuling pill decoction.</p><p><b>METHOD</b>The best extraction condition of the semi-bionic extraction for Guizhi Fuling pill decoction was optimized by uniform design was optimized with paeoniflorin, cinnamic acid, laetrile, pachymic acid, total polysaccharose and dry extract as the indexes.</p><p><b>RESULT</b>The optimized extraction condition is that extracting for 3 times with solvent pH values of 4.98, 7.35 and 8.00, respectively, and the total extracting time was 4.99 h.</p><p><b>CONCLUSION</b>In consideration of manufacturing fact, the pH value of thrice extractions is set at 5.0, 7.5, 8.0 respectively, and the extracting time is 2.0, 1.5, 1.5 h in turn.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To optimize the extraction condtions in the modification of Rhizoma Cyperi, which had been extracted by the technique of supercritical CO2 extraction.</p><p><b>METHOD</b>The semi-bionic extraction (SBE) conditions were optimized through homogeneous design while the extracts (< or = 1 000), the total area of HPLC and dry extracts were adopted as markers.</p><p><b>RESULT</b>The optimized SBE extraction conditions are the following: pH in first extraction was adjusted to 2.005 3. And then pH in second and third extraction adjusted to 6.508 2 and 8.945 6, time of the whole extracted process was 3.912 7 h.</p><p><b>CONCLUSION</b>Combine the faction of production,we make sure the conclusion is pH in first extraction was 2.00, pH in second and third extraction adjusted to 6.50 and 9.00, duration of run was 2.0 h, 1.0 h and 1.0 h, respectively.</p>

ABSTRACT

Traditional Chinese medicine(TCM) injection,which plays a unique role in the clinical treatment,is Chinese original dosage form.But in recent years due to its quality problems or unscientifically clinical use,some adverse reactions have reported,even several injections had serious adverse reactions.So how to develop TCM injection becomes one of the topics the public and scholars most concerned about.Therefore,the three "bottleneck" problems of TCM injection on the current development,which are the material basis and TCM theory,clinical efficacy and safety,and also quality control,were analyzed and discussed in the article.Then the countermeasures were proposed and provided ideas for future research and development of TCM injection.

ABSTRACT

Objective:To examine the expression of integrin ?V?3 in periodontium during experimental tooth movement, and then to propose the possible role of ?V?3 in bone remodeling during tooth movement. Methods:Thirty-five rabbits were divided into 7 groups (5 rabbits per group):untreated(control), 1,3, 5, 7, 14 and 21 d groups. A elastic coil spring was ligated between the incisors and the first molar and exert a force of approximately 0.78 N to pull the first molar mesially. The animals were sacrificed at each time intervals(1,3,5,7,14 and 21 d) respectively and specimens were made to observe the expression of integrin ?V?3 in periodontium through in situ hybridization. Results:Different extent expression of integrin ?V?3 were seen in osteoclasts, osteoblast precursors, osteoblasts,odontoclasts and odontoblast except bone cells. Conclusions: Integrin ?V?3 may play an important role in bone modeling, cementum resorption and its reparation during experimental tooth movement.

ABSTRACT

Objective Comparing the serum fingerprint of four kinds of extract solution from Banxia Baizhu Tianma Decoction.Methods Taking rabbits as experiment animals,collecting blood,and separating serum after drenching semi-bionic enzyme extracting(SBEE) solution.HPLC was used to detect the fingerprints of medicated serum at different blood-collecting time in order to optimize the best blood collecting time,then the serum fingerprints of four kinds of solution collected were compared.Results HPLC showed that when blood collecting time was 2 h,the number and total area of chromatographic peak had the maximum value;SBEE Solution had the the maximum number and total area of chromatographic peak in the four kinds of solution at the best blood collecting time.Conclusion The component content in SBEE medicated serum is higher than that by other method.

ABSTRACT

Objective:Optimising the technological parameters for the extraction of Maxingshigan Decoction (Herba Ephedrae, Semen Pruni Armeniacae, Gypsum Fibrosum, Radix Glycyrrhizae). Methods:Using the even design and TCL, ephedrine, glycyrrhetinic acid, hydrocyanic acid, calciumion and extract weight were taken as the indexes. Results:Optimal parameters: the pH values of the water for the 1st, 2nd and 3rd decoctions are 2.0 ,6.5 and 9.0,respectively; and the time of 3 decoctions are 2.0,1.0and 0.5h, respectively. Conclusion:Optimal parameters for Maximpshipan Decoction are feasible in extraction.

ABSTRACT

Objective: The extract technologies for the components of Fuzhuang Capsules were optimized. Methods: Four methods: the semi bionic extraction (SBE), the water extraction (WE), the semi bionic extraction by precipitation with alcohol (SBAE) and the water extraction by precipitation with alcohol (WAE) were used to extract Fuzhuang Capsules, with panaxadiol, panaxatriol, ? sitosterol, icariine, betaine, total sugar and dried extractive taken as the marker, under the same conditions of drug granularity, solvent amount, decocting temperature, filtration, concentration, etc., and then four methods were compared and studied. Results: Through the comprehensive evaluation of seven markers, their comprehensive values Y decreased in the order of SBE, WE, SBAE and WAE. Conclusion: SBE is better than the other three methods in the extraction of the components of Fuzhuang Capsules, which is showed by the fact that the pH values of the water for the 1st, 2nd and 3rd decoctions are 3.4, 7.5 and 8.4, and the extraction times are 2.0h, 1.5h, and 1.5h respectively.
